The Ballad of Gay Tony refreshes Electro-Choc, Vladivostok FM, K109 The Studio and San Juan Sounds with 52 new songs, and the Episodes from Liberty City disc adds Vice City FM, RamJam FM and Self-Actualization FM with 48 more.
The Ballad of Gay Tony keeps GTA IV's radio but gives it a nightlife makeover to match Luis Lopez and Tony Prince's club world. The downloadable episode refreshes five stations, four of them with new playlists (52 songs in total, several with new DJs):
- Electro-Choc: 13 songs, with Crookers behind the decks.
- Vladivostok FM: 15 songs.
- K109 The Studio: 16 songs.
- San Juan Sounds: 8 songs, with Henry Santos taking over from Daddy Yankee.
- Integrity 2.0: Lazlow's talk show gets new segments.
The standalone Episodes from Liberty City disc, released on Xbox 360 on 29 October 2009 together with the download (PlayStation 3 and PC followed in April 2010), adds three exclusive stations with another 48 songs: Vice City FM, RamJam FM and Self-Actualization FM. On the disc they stand in for GTA IV stations that did not carry over, and the updated stations play only their episode material; with the download on top of GTA IV, the new tracks simply join the original rotations. Below are only the new songs; GTA IV's own playlists are on the GTA IV page. Some tracks were removed in April 2018 when their ten-year licences expired.

Integrity 2.0
Lazlow's street-interview talk show returns with new segments and an illegal immigrant sidekick, Jorge. Lazlow yells at a street artist who draws him with "man-tits" and a bald patch, gets chased by a policeman for urinating in public, and is beaten up by a street musician after standing in his donation pile. Turned away from Maisonette 9 and unable to find Bahama Mamas, the pair end up in Hercules, seemingly unaware that it is a gay club. Since the episode takes place halfway through GTA IV's story, the station is available from the start.
Vice City FM
1980s pop, exclusive to the Episodes from Liberty City disc (and later the PC version of GTA IV: The Complete Edition), where it replaces The Vibe 98.8. The host is Fernando Martinez, who ran Emotion 98.3 in GTA Vice City and Vice City Stories. Evan Moss sings along to "Maneater" at the start of the mission Bang Bang. Five songs were removed in April 2018:
- "Wood Beez"
- "You're the Voice"
- "Teardrops"
- "History"
- "C'est la Vie"
- "Maneater" - Hall & Oates

Self-Actualization FM
Ambient and chill-out, hosted by Audrey: despite the name, it is a music station, not a talk show. It replaces The Journey, Fusion FM and Jazz Nation Radio 108.5 on the disc, and its playlist is newer than The Journey's, from the late 1980s to the late 2000s. The chatter between songs ties it to the Children of the Mountain cult, with mentions of Brother Adrian.

There was no radio soundtrack album for the episode, but its theme song did come out on record. "I Keep on Walking" by Aaron Johnston, Jesse Murphy and Avi Bortnick is the episode's theme, and one of the few GTA themes with lyrics. It appears, together with a downtempo mix, on Grand Theft Auto IV - The Theme Song Collection, released on 13 April 2010, the day Episodes from Liberty City came out on PC in North America.
